Introducing the Pediatric Palliative Care Collaborative
In 2009, three nonprofit agencies, Coastal Kids Home Care, Children’s Hospice and Palliative Care Coalition, and Jacob’s Heart Children’s Cancer Support Services joined forces to provide specialized pediatric palliative and end-of-life care to dying children in Monterey and San Benito Counties where no hospice agency was willing or able to care for kids. As the lead care coordinator for the group, Coastal Kids oversees the bundling of services that include in-home nursing, social work, care coordination, transportation to and from treatment, massage, music and art therapy, anticipatory and post-death grief support, physical and occupational therapy, and emergency financial and funeral assistance for low-income families.
Coastal Kids’ team works in close partnership with each child’s primary care physician, community-based resources, and with specialists from Lucile Packard Children’s Hospital at Stanford and the University of California Medical Center in San Francisco, to deliver the best possible care at every stage of a child’s illness and recovery.
Coastal Kids Home Care also works with California’s Department of Health Care Services as one of the few approved pilot site providers for the Medi-Cal Pediatric Palliative Care Benefit.
